The purpose of the questionnaire is to provide you with important information in regard to the responsibilities you have under the Duty of Care legislation

^3. Do you know the definition of Clinical (otherwise known as controlled) Waste?

^4. Do you know your responsibilities in regard to the Environmental Protection Act 1990?

^5. Do you fully understand your legal responsibilities in regard to Duty of Care?

^6. Do you know the difference between a Carrier's Licence and a Waste Transfer Station?

^7. Does your current washroom services provider hold a valid Waste Carrier's Licence?

^8. Is your current supplier a registered Waste Transfer Station (WTS)?

^9. Are you given a signed copy of the Waste Transfer Note on every service day?
